Physicals & MIAA Participation Forms

As the preseason approaches, please be sure to take care of all the necessary paperwork that was distributed during the fall sports meeting. Each year, we receive notice from the athletic director that players have either not yet submitted a physical exam or submitted a physical exam which is not current enough to be accepted for the fall season. Missing or expired  physicals will prevent players from participation (including tryouts). This can be very disappointing and stressful for both players and coaches. Please update your status through the athletic director before the preseason begins. You may also choose to place the forms in my school mailbox (as many of you have already done). I will then forward the information to the nurse and athletic director.
